The bulk of your menstrual flow is the uterus lining - mostly this sheds as individual cells so mixed amongst the blood it cannot be seen, but during heavier flow you may also notice larger pieces of uterus lining that has been shed. Some people refer to these larger pieces of uterus lining as 'clots' or compare them to pieces of raw liver.

There is of course blood too, this is similar to the blood that you would see if you cut yourself only menstrual blood doesn't clot like normal blood. As the blood passes out of your cervix it mixes with cervical mucus so you may notice big gooey lumps of blood, and along with that as it exits your vagina it will also carry along with it vaginal discharge.
