It is when you have an intrusive thought that won't get relieved until you act on it. For example, "If I don't wash my hands right now, I will get a deadly disease that I can spread to my whole family..." This causes a person with OCD to act on their thought, hence the obsessive COMPULSION aspect. Even though the person can rationally think that they don't have to wash their hands that exact second, the overwhelming thought compels them to so that they can get temporary relief.
