It usually means you have a leaking valve cover gasket.
It doesn't necessarily mean a bad engine but with this mileage, you might as well go for a rebuild since if you just do the head it porbably will start using oil.
Assuming you mean engine coolant is mixing with the engine oil, you probably have a bad head gasket and or a cracked head. If you run that engine in that condition for very long, you will destroy the engine internals.
There is water in the oil, it sounds like a head gasket. Remove radiator cap and start engine, if the coolant bubbles its a head gasket.

A cracked radiator will NOT put water in the engine and make the oil milky. A bad head gasket WILL put water in the engine and make the oil look milky. That does not neccessarily mean you have a bad engine. Have someone fix or replace the radiator and replace the head gasket. It could be a good engine for a long time yet
