The evening before Jesus was killed, he shared a meal with his followers. At this meal he broke and shared a loaf of unleavened (ie flat) bread with them and as he shared it out he said "this is my body which is given for you, do this as often as you eat it in memory of me". After that he passed round a cup of wine and shared it with them and as he did so he said "this is my blood which is shed for you, do this as often as you drink it in memory of me".
Thus when Christians come together to worship they share bread and wine at the alter as Jesus instructed. The bread and wine are the "Sacrament" and the service of worship in which this is done is called the "Holy Eucharist" or "Holy Communion"
