10mg is the strongest retail dose available at the moment although it can be compounded in a 15mg dose by a pharmacist on a doctor's special Rx order. The 15mg specially compounded dosage is very rare and almost never prescribed as the 10mg dose is usually just dosed at a closer interval to prevent "breakthrough pain". The highest dose that is recommended is 60mg of Hydrocodone in a 24hour period. This is the general rule. Recently the DEA and local law enforcement have "persuaded" (intimidated) doctor's in many U.S. states to prescribe no more than 4 tablets of ANY strength of hydrocodone a day. So for a chronic pain patient with moderate pain, it is common to receive a Rx for 120 hydrocodone tablets of whichever strength (5mg,7.5mg or 10mg is appropriate for the pain. This leaves us with a maximum daily dosage of 40mg (4 x 10mg tabs) for a patient in moderate pain. If the pain worsens the doctor most likely will not give the maximum allowed by law because of fear of being labeled an "OVERPRESCRIBER" and will move the patient up to one of the "STRONG" opioid painkiller such as Oxycodone, Hydromorphone, Oxymorphone, Fentanyl, or Morphine.
But in most cases the patient is given a non-narcotic to boost the hydrocodone instead of moving the patient up to the stronger drugs, even if they are appropriate for the pain.
