This question can send you in circles of laughable questions. Truth be told, I had to call my mom (she is like a thesaurus) and ask her, because thinking about it gave me a minor migraine (just kidding). But seriously, here is the answer I believe you are searching.
If your grandma has a sister, she is called your "great aunt". So her aunt would be your "great, great aunt". Sounds bizarre, I know, since your grandma is only your grandma and not your "great" grandma. But anytime an elder family member is removed (from different generations) from you, they are referred to as "great". Each additional removal merits another "great". I'm not sure this applies to cousins and such...most people say "my cousin, twice removed", meaning they are from the same great-grandparents, but not the same grandparents.
This does not mean you need to call her "great, great aunt Judith", or whatever...this is only the proper title. I'm sure she would appreciate just being called "aunt".

She is a great-great aunt to one of your parents and a great-great-great aunt to you
He is still an uncle, but more specifically he's your great uncle.
Your grandfather's sister is your great aunt. Some use the term grand aunt.
