It really depends on what you are looking for in a cruse ship. Think of cruise ships in two categories.
The first is the mainstream category that includes Royal Caribbean, Carnival, Princess, Holland America, Norwegian and Celebrity. The top two here for Alaska are Celebrity and Holland America. You get a combination of nice elegant ships and top notch service.
The second category of ships are the luxury liners like Regent Seven Seas or Crystal cruises. These tend to cost more but than the mainstream lines but for one price you get food, drink (including alcohol), airfare, excursions and even pre-cruise hotels. I'll add a link in the related link section that gives a good breakdown comparing the two categories in Alaska. For the best line in the luxury category I'll go with Regent Seven Seas.

The Carnival Cruise line is a great cruise line for going to Bermuda. There are also other cruise lines that go to Bermuda such as Royal Carribbean and it depends on the ammenities needed for the cruise.
A Fly cruise is a cruise that you have to fly out to get to the port. For example, if I, a resident of the UK, wanted to go on a cruise to Alaska, instead of getting a boat from here to Alaska that would take me a week at least, I can fly out to Alaska and get on a cruise which leaves from there.
This depends on where you are planning to go. Carnival Cruise Line is fairly common and can take you to Mexico, Europe, South America and Alaska, to name a few.
