The best way to find all of the instance entrances is basically to visit them.
There are three instances located in Uldum. The Lost city of Tol'vir, The Vortex Pinnacle, and The Halls of Origination.
Lost city is located on the east side of the map in a pyramid.
The Vortex Pinnacle is located on the bottom of the map towards the SE of the bottom.
The Lost City of Tol'vir is located just in front of the Vortex Pinnacle.
The Throne of Tides is located in the Abyssal Depths. You will see what looks like a crack or crater to the North of the starting area. The best way to get there is to swim to the surface and mount a flying mount.
The Stonecore is located in Deepholm. Upon entering the area you will be in the middle of the map in a giant Spire. Leave the Spire and head right, The entrance is located on the spire high up. This will require a flying mount.
Grim Batol is located in Twilight Highlands. Head far south from the first beach head and go to the edge of the map. You will see what looks like an arch, the instance entrance is located in there.
Blackrock Caverns is probably the easiest one to locate. There is a quest to complete in Mount Hyjal that will take you right to the entrance.
You will not need to discover the entrance to Shadowfang Keep or Deadmines as these are updated version of older Warcraft instances
You get them by doing arenas mainly. For example, you get the title "Gladiator" by being the top .5 best arena teams

Mount Hyjal will be one of the new zones in the upcoming expansion Cataclysm. It will have a brand new dungeon, Firelands. This dungeon will be both 5 man and 10/25 man dungeons. Mount Hyjal was previously accessible to players, but later was closed off. I will be controlled by three factions. They are The Black Dragonflight, The Guardians of Hyjal, and The Twilight's Hammer Clan. Note: This information is considered sketchy at best as Blizzard may decide to change things before Cataclysm's release.

In absolute terms, the most money can be gained by clearing Stratholme and Scholomance on your own. Time for money seems to be best on solo'ing old world raids, however, but this requires you to be a class that is good at solo'ing content.
