1st you start up the car. Hook up a voltmeter to the battery, and put a load on the battery as the car is running (have the lights on, radio on, high beam on,...) after you put a load on the battery read the voltmeter. if it goes below 12 the alternator is bad. It souldn't be no more than 14 volts. you can also use the battery gauge but to be 100% sure hook voltmeter to the alternator.

Either the battery is bad and won't accept a charge or the alternator is not putting a charge into the battery. Have the battery tested. You can hook up an inexpensive volt meter to the battery while the car is running. You should see from 13.5 to 14.5 volts if the alternator is working properly. If it is less than 13.5 volts then the alternator is not charging.
Sounds like the alternator is not charging the battery up / WORKING check charging system. Charge battery to 12.8 volts and then start engine and hook volt meter to battery and it should read 13.8 to 14.3 volts. If it don't then replace alternator. Also need to make sure the alternator belt is tight.

You can start a car without a battery if you hook up a jump pack or use jumper camples but it could fry your alternator. It is pointless to charge a car without a battery. When you charge the car you are charging the battery.
Hook positive up first, negative last.
Voltage regulators are often built into the alternators now, this wasn't always the case. Older vehicles had separate voltage regulators mounted on firewalls or the side of the engine compartment, wired in between from the alternator to the battery. The positive lead now goes directly to the battery from the alternator/regulator.
