Creams are an O/W (oil in water) emulsion. Creams contain a small amount of oil dispersed in water. Most creams are "vanishing." They do not show once applied and are more cosmetically acceptable to most patients.
Ointments are an W/O (water in oil) emulsion. Ointments are considered semisolid. Examples of bases used in ointments are: OLEAGINOUS or greasy based made from hydrocarbons such as in mineral oil or petroleum jelly. Other examples of W/O emulsions are lanolin or cold cream.
Some topical products require special precautions for proper application and desired results. For example: Capsaicin cream (active ingredient is hot chili peppers) must be applied with gloves because severe irritation can occur if the cream is accidentaly rubbed into the eyes. (Used for Arthritis pain) Topical corticosteroids (used for itching, poison ivy, eczema etc.) are applied sparingly to the affected area for short periods of time. Only some steroids should be applied to the face. (ask your doctor or pharmacist for advice) The treated area should not be covered with a bandage or other dressing (diaper) because occlusive dressings can significantly increase drug absorption and the risk of side effects. Overuse or inappropriate use of potent topical cortiosteroids can lead to serious systemic side effects.
