Identical twins would look the same, fraternal twins could be of different sexes. In the womb, you can easily tell the difference between fraternal and identical twins because if the twins are identical, they will share the same sac. Fraternal twins will have different sacs. You can see this in the ultrasound. After birth, it is often very difficult to tell the difference between the two types of twins because babies often look alike particularly when they are siblings even if they aren't identical genetically.
