We assume you mean the difference between an addict and a non-addict.

When people are exposed to drugs for prolonged periods, their brains undergo changes and adapt to the presence of the drugs. When that happens, the person needs the drug in order to feel something like normal, and they are addicted. In order to get high, they have to take more and more of the drug as this tolerance builds. Tolerance is one of the first signs of addiction to any drug, including alcohol.

When the drugs are removed, the changes in the brain scream out for more drugs. The brain has adapted, and cannot go back to normal quickly. In order to recover, addicts must abstain from all drugs for long enough to allow the changes in the brain to reverse (to the extent that they are able to) so that they can again feel normal without the presence of the drug.

Unfortunately, not all of the changes reverse, which means that if an addict uses again, it will take very little time for them to be back to the point where they were before, and the process will have to be repeated -- if they don't die first.
