When you are using a communication medium (plural: communication media), that refers to sending out (transmitting) information or messages by using radio, TV, newspaper, the internet, movies, magazines, etc. A communication device is something you can receive those messages on: for example, I can use my smartphone to read the news or post a comment about it to Twitter.
