Why, it's Slappy, of course. He's still annoyed at you for beating him up on the first map. If I remember correctly, you'll have to get through the Mirror Maze to fight him. I've heard there's a glitch with this Map though. Not many people actually make it to the fight because of the Mirror Maze. Apparently, it's never ending, for many people at least. That's what I've heard though, I haven't tried the last map myself yet.
If you do make it to the final fight with Slappy, you basically have to drain his health, and get him into the mirror that leads to Panic park. Do that, and you've beaten the game, assuming you collected all of the other tokens in the other maps, that is.
