Okonkwo's downfall is his inability to completely follow his tribes traditions, and then his inability to adapt to European colonization.

3. Okonkwo's accidental killing of a tribesman.
4. Okonkwo's killing of the messenger.
5. Okonkwo's suicide.

Eventually the Oracle of the Hills and Caves would pronounce that he was to die. He would be led out of the village and attacked, the final blow coming from Okonkwo himself.
When Okonkwo's life and status starts dwindling, and the final fall is when he dies.
The young virgin girl is sent to be Ogbuefi Udo's wife as a replacement for Ezeugo.
The girl was given to the man who's wife was the one killed, but we never heard about her again. And the boy, Ikemefuna, was given to Okonkwo as they figured out what the land wanted to do with him. After three years the oracle decided that Ikemefuna needed to be killed.
Fate was both their meeting, which brought them together, and who their families were, which tore them apart. There isn't a really complex answer for this one.
