1. Physical sex - This what genitals a person has, and the reproductive role the person would play.

2. Gender identity - This is a pervasive and lifelong sense of who someone is. This ties somewhat into interests, abilities, priorities, and core nature. It is who you are.

3. Gender role, or gender expression - This is all the learned aspects of gender and all the social rules. It is about what you wear and how you act.

4. Sexual orientation - While mostly irrelevant in this context, orientation is here because people confuse someone's sexual preference with gender identity and gender role. A male-bodied person thinking and feeling like a woman doesn't make them want to have sex with men, neither does dressing and acting as a woman.

Colloquially, you could think of physical sex as the Plumbing, gender identity as the wiring, gender role as the paint/appearance, and sexual orientation as the heat.

Earlier versions of this model only had 3 elements. The problem with that was that it didn't explain feminism and recreational transgenderism that didn't result in surgery. The 3-part model left the term "gender" up to interpretation, so persons born with transsexualism and the need to conform within the gender binary could use the term to mean gender identity or neurological gender, while transgender and feminist persons would use gender to mean a person's social role and the gender construct of society. But with this model, it can differentiate between a person with a body at odds with their nature (transsexual), and someone whose preferred social role was at odds with their body and with society (transgenderism, radical feminism). In the transsexual person, their body is at odds with their nature, and the emphasis is on fixing the body. But for the non-op transgender person, the emphasis is on the social role and passing, and they take little or no issue with the body, thus no need for surgery. You don't get genital surgery if there is nothing wrong and nothing to improve. Transsexual persons tend to blame their body and take steps to fix it, while transgendered persons see gender more as a construct and place the blame on society. Thus they align with the LGBT and become activists to try to change society.
