Because it got really emotional between Oliver and James, to them it felt like the real thing. So Oliver quit after 5 takes because the scene emotionally drained him and he was about to break down. For him it felt like James was lying dead on the ground, not Fred.
The answer for this is given in the script of the last ever episode, for the final scene, where Lynda appears to Spike in his apartment. it is explained in the script (as instructions) that ''It takes a moment to impact on Spike that this is no dream and that she is alive'' and also that ''He has come over to her, touches her. He is hardly able to believe that she's real''. This confirms that Lynda survived and didn't die in the fire that was featured in the episode. This scene was deliberately made ambiguous as to avoid a patronising cliche of 'and they all lived happily ever after'.

The last scene shown is the scene in which Vincent and Jules get caught up in the robbery of the restaurant. In the established chronological order, the last scene is when butch and fabian ride off on zeds motorcycle.
