Not Saw. Try Tokyo Gore Police, Planet Terror, Rambo, Braindead and Philosophy Of A Knife (unrealistic but horrific).
It has some of the most realistic gore and blood that I have ever seen in a film. One of the selling points to this franchise is the hardcore showcase of violence and pain.
The gore in the movie Misery is quite graphic, not as near graphic as the novel, however. The most brutal scene is the ankle snapping scene. You may not want to watch that part although it is quite brief.
