Form 4506-T is Request for Transcript of Tax Return. You can print online at www.irs.gov/formspubs by clicking on 'Form or instruction number'. It's a one-page form. The first five lines ask for personal information to identify your tax return. For example, the name shown on the tax return, social security number, your current address, and address shown on previous return, if different from current. On Line 6 indicate the main tax form for the transcript request (i.e., 1040, 1065, etc.). Boxes 6a, b, and c offer the choice of type of transcript: return, account, or record of account. Check Box 7-Verification of Nonfiling if you're requesting proof that you didn't file a return for that year. Check Box 8-Form W-2, Form 1099 series, Form 1098 series, or Form 5498 Series Transcript if you want data from any of those forms included in the transcript. On Line 9 enter the years that you're requesting. Be sure to fill out the signature section at the bottom of the form.
Form W-2 and Form 1099-R (What to Do if Not Received)If you do not receive your Form W-2 or Form 1099-R by January 31st , or your information is incorrect, contact your employer/payer. If you do not receive the missing or corrected form by February 14th from your employer/payer, you may call the IRS at 1-800-829-1040 for assistance. You must provide your name, address (including zip code), phone number, Social Security Number, dates of employment, your employer/payer's name, address (including zip code), and phone number. The IRS will contact the employer/payer for you and request the missing form. IRS will also send you a Form 4852 (PDF), Substitute for Form W-2 or Form 1099-R.
