The Preamble of the United States Constitution includes that phrase. "We the people of the United States, in order to form a more perfect union, establish justice, insure domestic tranquility, provide for the common defense, promote the general welfare, and secure the blessings of liberty to ourselves and our posterity, do ordain and establish this Constitution for the United States of America."

It actually says, "...more perfect Union". Lincoln recognized that the Union was not perfect. The new Constitution was designed with the goal to IMPROVE the Union, making it "more perfect".
"To form a more perfect union" is important because it is the goal of the Constitution. The Union was poorly organized at the time of the constitution.
This is not from an amendment but the preamble to the constitution. " we the people in order to provide a more perfect union........"
