Applied. The general principle is that if a verb ends in y, the y is changed to i in the past participle.
Apply is a regular verb so the past and past participle are both verb + -ed. You have to watch the spelling because "the y changes to i then you add -ed". applied
The past participle of do is done. The past participle of have is had.
The past participle of am is been. Not does not have a past participle

To isn't a verb and so doesn't have a past participle. The past participle of be is been.
