If you work at a hospital, you will need to haggle with HMO's to get reimbursed. On the other hand, if you run your own business, you decide what insurance (if any) you will accept and all the cash goes directly to you. Keep in mind that no matter what route you take, if you want to be a doctor just for the money, you're doing it for a wrong reasons and you won't last a day in residency, let alone med school.
