The only place astronauts have been other than Earth, is on the Moon. Between 1969 and 1972 there were 6 moon landings. Nobody has gone to the Moon since then and nobody has gone to any other planets yet. Most astronauts now just orbit the Earth on one of the Space Shuttles or on the International Space Station or one of the other spacecraft that some countries have. It will be a long time before it will be possible to send astronauts to other planets.

Astronauts do not generally discover planets; astronauts are people who go to space, but we have never send a manned flight past our own moon (which isn't a planet). So barring the somewhat lame answer that it is the Earth itself, the answer is none.
If you ask which of the planets in the solar system was discovered in general then it was Neptune in 1846. Several dwarf planets (including Pluto) have been discovered later and over the recent years.
By the way, there are also planets orbiting other stars, in other solar systems that are being discovered these days.
