You can send your bank a letter through the mail, or deliver it to them in person. They do need a signed letter to change your address, since your financial records are confidential and it would invite abuse if they just changed addresses on the basis of a verbal request, that could be coming from anyone.
If you are not sure of what needs to be done, call up your banks customer care. They will direct you appropriately.
