You'll need a note from your doctor ( your family doctor or primary care physician will be fine) stating that the placard is medically necessary, and the reason could be as simple as Asthma. Then simply present this at the DMV, where you will be charged a nominal fee for the placard. As for the sleeve, I wouldn't recommend it. They tend to yellow and crack and ruin the placard.

The only legitimate reason to park in a handicapped designated parking place is that you, or one of the passengers at that time, is handicapped and the vehicle was displaying a handicapped parking permit.
