Well... At first you don't really know but as the story progresses you kinda get a hint. Akito's mom raised her as a boy because she didn't want to believe that she had given birth to the god of the zodiac. Also In a couple of volumes (not reall sure which) you see she has a relationship with Keureno her supposed lover. Also In the Manga you see her wearing flowerd komonon's. In the anime on the other hand they portry Akito as a boy (for reasons I don't get). At first in the manga the only one's who knows Akito's true gender Hatori, Shigure, and Kuereno.
