Formally for the envelopes on the letters :: "Mr. and Mrs. William Smith, Jr. "
informal ... Bill and Peggy (for when you are greeting them in the receiving line at the wedding.)

The name followed by Jr For example: John Doe Jr
If the Pastor who is the third and not a Junior is the only Pastor then you address him and his wife: Pastor & Mrs. John Doe. If all three were Pastors then you would address the letter as: Pastor John Doe,III & Mrs. John Doe.
The proper way to list a husband and wife when his last name has a Junior behind it is: Mr. & Mrs. John Doe, Jr.

Like the words mom, dad, uncle, etc, wife would be capitalized if it is used as a proper noun, like a name. If it is not a proper noun, it is not capitalized. For example, "Wife, bring me my slippers" vs "I would like my wife to bring me my slippers"

Do NOT, repeat NOT, use the widow's first name. You address the envelope just as though the husband were still alive. Example: Mrs. Robert Smith While that is the formal way address, in today's society, many women prefer to be addressed with their own first name. Neither is considered incorrect.
Assuming a business letter to husband and wife with different last names who you don't know the proper business salutation is to have the the wife's name first before the husband's accompanied by the proper titles. For example: Dear Mrs. Thompson and Mr. Jones, (salutation on letter) Mrs. Susan Thompson and Mr. Jeremy Jones (address on envelope and letter) Note that you could use "Ms." instead of "Mrs." if you prefer, although I recommend that since in this example you know they are married I would choose "Mrs." Jim Lumley jimlumley@live.com
Say there last name was "Smith" you would write Mr. and Mrs. Smith.
Mr. and Mrs. James J. Doe If writing to him alone, the correct form would be James J. Doe, Esquire
All proper nouns gets capitalized in an address. Example: 2021 Smith Street, Malate, Manila, Philippines
