Skin effect is a physical effect that operates on thick conductors. In a transformer the conductors are usually too thin for any substantial skin effect to occur.
If the voltage drop is excessive, it is a sign that the circuit conductors may be undersized. Better to install larger conductors. If you try to use a transformer, under light loads the voltage will rise to unacceptable levels, possibly damaging equipment.
Whether you have an overhead or underground feed, that section of the installation is referred to as Service Entrance Conductors. On an overhead installation, it includes the conductors on BOTH sides of the meter, from the service drop conductors (attached at the service head) to the service equipment LINE terminals. On an underground installation, it refers only to the conductors between the meter's LOAD side and the service equipment LINE terminals. The conductors on the LINE side of the meter come from a distribution transformer and are unbroken between the transformer and your meter.
Generally speaking high-voltage three-phase systems are three-wire systems comprising three line conductors, so the high-voltage primary of the transformer is delta-connected (a delta connection has only three terminals, to which the three line conductors are connected). European low-voltage three-phase systems are four-wire systems, comprising three line conductors and a neutral conductor. To obtain this, the transformer's secondary must be star (wye) connected, with its earthed star point providing the neutral.
