It depends on the substance you've ingested, or the reason you're tripping in the first place. Psychedelics (LSD, etc.) have different feelings and mental states associated with them than Dissociatives (Salvia, etc.).
Psychedelics tend to leave you more "in reality" while producing visionary and hallucinatory effects on top of it (which can still very easily leave you confused and/or scared about what is going on). You can typically still recognize those around you and communicate in basic ways, however.
Dissociatives tend to completely wipe out reality and take you to another place, very similar to a dream-like state. You will often not recognize those around you, or even realize they are there at all. It is very easy to injure yourself in this state and not even realize it, as the real world is essemntially removed from your psyche temporarily.
The feeling of really tripping balls (not just seeing tracers and brathing walls, but really all-out tripping balls) is kinda like your brain being rewired. It's like, you see things that may or may not be there, but they're extremely vivid and lifelike, so it's easy to convince yourself that it's real. Colors tend to be amplified and often your "edge-finder" in your brain (spacial recognition, etc.) will feel "turned up."
Used for introspection and inner learning, these substances can be interesting and even enjoyable if used carefully and with proper safety in mind. The experience can easily become terrifying and unpleasant, however, especially if in the middle of the trip you fail to realize that you've taken a substance which is causing you to feel this way -- it may feel like you're going to stay that way forever and that can be very upsetting.
