If you mean in RuneScape, both are quite real.When talking about somebody's "level", you need to specify whether you mean total levels, or combat level.
The highest possible combat level is currently 138; so if somebody mentions a higher level, it probably refers to the total levels.
Levels up to 138 MAY refer to combat levels; or it's the total levels of a player who hasn't trained much yet (since it's relatively easy to get a total level above that).
