As far as i know, the actual phone has to have 4G hardware in it and a 4G antenna to communicate with the upgraded 4G tower equipment. That's pretty much it.
On the more technical side, 4G uses an entirely different air interface to communicate from mobile to base station.
Yes. Straight Talk has a number of 4G phones available. Many times you can also bring your own 4G compatible phone.
No. A 4G phone typically is compatible with 3G networks, especially since 4G networks are not entirely established. 3G however is a lower generation technology that was created prior to the 4G so there just isn't a way to have the technology already in place for the newer.

The Samsung Dart is not a 4G phone, it is a 3G phone. You will not be able to use it to connect to a 4G network. You'll need a 4G phone to connect to a 4G network.

The earlier models of iPhone were produced when 4G was not out yet and so, obviously, they cannot be compatible with it. However, since 4G has been released, you can expect the newer models to be 4G compatible, especially since the current iPad is capable of using some 4G connections.
