There is no direct metro train and you will have to change lines to get from Madeleine to Gare du Nord. One option: start at Madeleine taking line 8 towards Créteil-Préfecture; change at Strasbourg-Saint-Denis for line 4, towards Clignancourt. Step down at Gare du Nord. You can't go on the metro, you have to go on the RER (google it). From the centre of Paris it takes about 35 minutes.
The entry is at the train station that takes you from Paris to the Disney site.
There is no Notre-Dame metro station. The nearest is Cité. It takes about 15 minutes from there to Gare du Nord. The Saint-Michel-Notre-Dame station (metro and RER) is slightly further from both.

Nice easy one. At Gare del'Est, down the steps to the metro station. Buy tickets (un ticket, s'il vous plait). Metro line 4, direction Porte d'Orléans. 12th stop is Montparnasse-Bienvenue. There is a moving pavement which leads from the metro station to the train station.
