Plagues usually attack through the water you drink if it is not filtered and also through other animals and people also through your food but in the U.S. you don't really have to worry because the worst think you could get is a parasite or a cold or something you are least likely to get the plague in the U.S.A.

It usually causes necrosis in the skin and bleeding of the inner ear, but other parts of the body can be infected. Ultimately, it causes death in many or most cases.
