I'm not sure that there is actually a percentage for this, but it is more common than some might think.

While it might be slightly humorous to think of someone dying on the toilet, due to the contraction/constriction of the blood vessels and muscles while someone is defecating, people can suffer from severe heart attacks and die.

(In a controlled setting, bearing down much like when a person is going to the bathroom can be used to lower pulse rates. This is called the Vasalva maneuver. When the person bears down, the blood vessels constrict a bit and help the body "naturally" lower its pulse.)
