I am not sure what you mean by "designing" it. MTV, originally called Music Television, made its debut in 1981 as a cable channel that featured music videos. Its founders were Robert W. (Bob) Pittman and Tom Freston (who later became the head of MTV after Pittman left). MTV has undergone a number of changes over the years, moving away from music videos and more towards features and dramatic programs aimed at the youth audience. Today, each of the programs on MTV has its own staff of men and women responsible for producing and directing and hiring the talent for each of the programs. Thus, I cannot say one person only "designs" MTV.
MTV 2 originally started as a platform from more music based programming including more music videos, as MTV 1 had started to focus on reality TV. Currently MTV 2 now shows programs such as Guy Code and Boy Meets World.
