The rank of a Coast Guard Helicopter Pliot varies. Because the pilot will have to have completed flight training, the minimum rank is Lt(jg) (O-2), but more than likely all pilots will have made full Lt (O-3) by the time they pin on their wings.
The highest rank for a pilot is typically a LCDR (O-4). By the time an officer is selected for CDR (O-5), they are typically not in the flight line. At the same time, provided they maintain their flight qualifications, CDR's can be pressed into flight service in emergent situations.
That rank has never existed in the Coast Guard. The highest rank in the Coast Guard is a four-star admiral referred to as "Commandant", just like the Marine Corps. The position and rank is currently filled by Admiral Robert J. Papp, Jr.

No, the equal rank in the Navy or Coast Guard is a Captain.
The Coast Guard uses the same rank structure as the Navy. Thus, anyone who wears the rank of Ensign, Lieutenant JG, Lieutenant, Lieutenant Commander, Commander, Captain, or any Admiral rank, in considered a commissioned officer of the Coast Guard, and is also recognised by the rest of the Armed Forces as a commissioned officer, as well.
In the United States Coast Guard, the rank CD-R stands for Chief Damage Controlman. This is a senior enlisted rank, typically responsible for leading and supervising damage control operations aboard ships or shore facilities. CD-Rs are highly trained in firefighting, shipboard safety, and damage control procedures.
A "non-commissioned officer" an enlisted man with petty officer rank.
Pay is dependent on rank and time in service. While the majority of Coast Guard members don't make $60K right after school, it is possible.

A Warrant Officer is a rank within the US and many other armed forces that is between the rank of a Non-Commissioned Officer (which is any of the sergeant or chief ranks) and an Officer (such as Ensign or 2nd Lieutenant) The rank of Warrant Officer was created so that individuals with certain technical specialties could be placed in a higher grade than the enlisted, but yet not have all the responsibilities of an Officer. For example during Viet Nam, many helicopter pilots were Warrant Officers. Today, many of the Army's helicopter pilots are Warrant Officers.
There is no kind of about it. Both the US Navy and Coast Guard use the Rank Master Chief Petty Officer for the highest enlisted rank. Only Warrant and Commissioned officers rank above Master Chiefs.
Military salaries are provided in a table. It is based on the time in service and the current rank. You can find the table at http://www.uscg.mil/ppc/docs/2015basepay.pdf
"SK" stands for the rate (Navy doesn't use the term, 'Rank') of Store Keeper. While the Navy no longer uses this rate, the Coast Guard still does.
