There is no Warrant Officer Grade 2 in the US Army. The ranks of Warrant Officer are as follows:
Warrant officer is its own rank/pay scale. There's WO-1 all the way up to WO-5.
They out rank the enlisted personnel and are in turn out ranked by the Commissioned Officers.

Chief warrant officer ranks go from rank W-2 to W-5. The warrant officer ranks are a division above the NCO ranks, which outclass the enlisted ranks within the military.

Yes and no. Sergeant Major is a position, not a rank, but it is always held by an E-9 enlisted man. A Warrant Officer wasenlisted, but is now an officer. He ranks below the most junior commissioned officer but above the most senior enlisted.
In the context of interviewing military officers for my writing of magazine articles, I've been told that I may address an officer by first name, but that it's always safest to address an officer by rank and last name, as in "Commander Davis."
