There are a few things that can dramatically reduce a reader's ability to understand what they are reading. Lack of sleep is one of the main culprits. Another thing is stress. When a person is under stress, their mind is not relaxed and they don't absorb the information they are taking in. Finally, they could be reading above their level. This would cause them to not fully comprehend what they are reading.
