If your muffler falls off in the middle of the road when you are travelling in a highway or busy road, best is to ignore it as you may risk your life otherwise. You can wait to pick it up if it is a local road and if there is not much traffic.
Pull over to the side of the road and turn on your 4-way hazard lights. If it is safe to do so, try to get the muffler off to the side of the road as well. If you are able to carry it, put it in your trunk and get to the nearest garage or muffler shop for repair.
