At that point, if you have not yet tied a tourniquet, tie one above the wound making sure it is tight, keep it at an elevation and keep adding direct pressure to the wound. Not much else can be done to stop bleeding. If you can see the blood vessels themselves you can use a cautery if you have one (It essentially uses heat to close off the blood vessel).

∙ 12y agoTry applying pressure to a pressure point. For the arms, this is the brachial artery in the armpit; for the legs, it's the femoral artery on the inside of the thigh, near the groin. If that fails as well, use a tourniquet--but ONLY AS A LAST RESORT. A tourniquet essentially is an overkill technique that stops blood flow to the affected area. It stops the bleeding, but lack of blood leads to gangrene (tissue death), and you might have to amputate the limb.

Make a tourniquet. But, if you're working for an ambulance company, check their protocols bc some companies have done away with tourniquets.
