Everyone is different but the usual signs are facial hair, pubic hair, deepening of voice, acne and body odor. One of the first visible signs of puberty in boys is that your testes and scrotum begin to get larger and hang lower. This happens so that your testes can hang away from your body because they produce sperm better if they a little cooler than the rest of your body. This change tends to occur before any penis or hair growth.

Different boys can go through the various stages of puberty in different orders. Some might notice darker hairs growing just above their ankles or on the backs of their arms as a first visible sign. Other boys might first notice some long straight fair hairs growing close to the base of their penis or in their armpits. Other boys might first notice that their voice starts to alter and sometimes sounds deeper and sometimes higher pitched. Another normal first sign is for a boy to start getting frequent erections and to find that he is suddenly developing an interest in anything to do with sex. An increase in the size of the testes which causes the scrotum to begin to hanger lower (balls dropping) is a frequent early stage in male puberty.
The average first sign to a boy is he notices pubic hair beginning to grow and his testes increasing in size.

Stage I
- Prepubertal appearance

- Pubic hair begins to extend from just above penis to more of a triangle shape

- Scrotum continues to enlarge
- Penis begins to enlarge, first in width, then in length (up to about 6 cm)
- Due to increased hormone secretion, 70% develop breasts temporarily
Stage IV
- Pubic hair has quality of adult, but has not extended to the legs
- Testicular volume about 12-20 ml
- Scrotum continues to enlargen
- Scrotum begins to darken in color
- Penis continues to enlarge, first in length (up to about 10 cm), then in width
- Due to increased hormone secretion, 70% develop breasts temporarily
Stage V
- Pubic hair extends to medial thigh
- Testicular volume 18-20 ml or larger
- Adult sized and colored scrotum
- Penis length around 15 cm
- Gynecomastia, if present, regresses
