Make sure they are all on the wall. Then change your view with the triangle button. Rotate each page until it looks like a map of the world. Start with the pages on the outside aligning the border. shouldn't take too much brain bustin'.

You can do this with any amount of pages you've found, but all 30 pages must be done when the congregation of assassins meed in the codex room much later in the game before leaving for Rome and the final stage.
