The Apollo missions which reached the moon were launched from the Kennedy Space Center. The space shuttle was not designed to reach the moon, but instead orbits the Earth at 200-300 nautical miles.
The first man-made object to successfully achieve earth orbit was Sputnik-1, launched by the Soviet Union (USSR) on October 4, 1957.
The last Space Shuttle missions flew in 2011, so they're no longer launched from anywhere. When they were, it was from the Space Center at Cape Canaveral, Florida.
