In the 1800s in the US most of the treatment of mentally ill was handled by religious groups like the Quakers. The main thing done was to provide order and reduce stress in their lives, this was actually very effective.

This system disintegrated in the 1880s with the mass waves of immigrants and was replaced by State InsaneAsylums, a system that remained until after the introduction of effective psychiatric medications in the 1950s. The main thing done in these Asylums was to confine them to prevent them from harming themselves or others, this was almost completely ineffective.
