It could be as simple as an occasional misfire so don't assume transmission problems. Start with the simple things first. Use a code scanner to see if there are any diagnostic codes that have been stored in the computer. That will usualy catch most electronic problems. And yes, it could be a fuel filter problem, but check the codes first. Yes it most likely is a misfire Check the plugs first then pull the wires off the coil towers You may find corrosion on the coil towers which will cause the misfire or jerk at higher speeds
You have a problem with your transmission. If it is only occasionally on - it could be just overheating. A dealer can retrieve the code to find out what the problem is.
I had the same problem with my 96 Grand Voyager. When the transmission has a problem it will lock in first gear as not to leave you stranded in the middle of nowhere. There are electronics in the transmission and sensors that have detected a problem. I hate to say this but the transmission has to be rebuilt or changed.

Have the codes read, my problem was with the throttle position sensor. My 94 le Sabre was also stalling occasionally.
