This is NOT true. Black holes are formed when massive stars explode in supernovas, blowing much of the star into space and crushing the core into a black hole. One of the things that WILL happen is a massive pulse of x-rays and gamma rays.
A black hole can't really form inside of another black hole. If you think of a black hole forming after a star goes supernova, then there isn't really a star to go supernove inside of the already created black hole. In fact, there isn't even any space inside of the blak hole for anything to happen. Two black holes can join together, but they wil eventually go to one.
The gasses ejected from the supernova would probably transfer enough momentum to the black hole to blast it away from the exploding star. The black hole would gain some mass from the gasses.

Population III stars more than likely created the first black holes, which became the supermassive black holes at the centres of most galaxies. They also produced the first "metals" into the new Universe.
No. A supernova is star that is exploding. If any planets are orbiting a star that explodes, they will be destroyed. There is evidence that after a supernova new planets may form from the debris cloud left behind and orbit the stellar remnant, which will be either a neutron star or a black hole depending on the mass of the star that exploded..
