Well, for a long time it didn't exist and people died. Yet, for many centuries most buildings weren't over 2-3 stories so people didn't need a ladder or fire escape. In the 1800's when steel was invented so was the multistory building and skyscrapers. In 1878 Joseph Winters used a wagon to hoist a ladder up a building . This led to ladders on fire engines. As buildings got higher and higher metal ladders were put on the outside of windows for people to escape.
