I believe that she and Firestar would have become very good friends... then perhaps more than friends, if Spottedleaf was okay with breaking the warrior code. They may have had kits. Sandstorm would probably have become Dustpelt's mate. However, if Spottedleaf didn't want to break the code, then she and Firestar would probably go through their lives ignoring each other, sort of like how Squirrelflight and Brambleclaw end up. Or they may have stayed just friends.

Spottedleaf is murdered in the very first warriors book, Into the wild, by a shadowclan warrior; Clawface. In the last book of the 4 series of Warriors, Spottedleaf's spirit saves Sandstorm from being killed by Mapleshade. Firestar grieves as Spottedleaf fades, she can no longer live in StarClan. Instead of meeting her in StarClan, Firestar loses his last life (Tigerstar attacked him, Firestar killed Tigerstar's spirit but still died from the injuries) and has to wait for Sandstorm to accompany him someday.
