I believe that she and Firestar would have become very good friends... then perhaps more than friends, if Spottedleaf was okay with breaking the warrior code. They may have had kits. Sandstorm would probably have become Dustpelt's mate. However, if Spottedleaf didn't want to break the code, then she and Firestar would probably go through their lives ignoring each other, sort of like how Squirrelflight and Brambleclaw end up. Or they may have stayed just friends.

If Spottedleaf was a warrior, she would likely have been able to participate more actively in Clan life, hunting, patrolling, and defending the territory. However, her skills as a medicine cat were crucial in many situations, so her absence in that role may have affected the Clan's health and well-being. Additionally, her close relationship with Firestar might have taken a different turn if they were both warriors.

Major changes would happen to the series. Spottedleaf would not have given Firestar prophecies. She would still be alive. Her and Firestar would be mates. Leafpool and Squirrelflight probably would have been replaced by different kits. Wow so much would have changed!

Instead of meeting her in StarClan, Firestar loses his last life (Tigerstar attacked him, Firestar killed Tigerstar's spirit but still died from the injuries) and has to wait for Sandstorm to accompany him someday.
