Could be the servo unit in the transmission. Had the same problem and replaced the fuel pump in our 1999 Ford Contour but it was the servo unit.
It could be your EGR valve.
The year, make, model and engine info would help, but I would start with replacing the fuel filter.
Basically, you don't. The filter is not easily available in the CD4E automatic transmission. It doesn't have a bottom pan, so it would need to be disassembled to get at the filter.
what would make a 2000 ford windstar die out at stop lights and you have to put your foot on the brake and gas at the same time to keep it running and it sputters and jerks.
Check your fuel filter.
It is located inside the Transmission You will have to take the transmission apart it is in the middle of the trany. It would be cheaper to get a new trany.

Could be a weak fuel pump or plugged fuel filter.
If the vehicle has a carburetor, check the rubber hose from the fuel filter to the carb. If it's old, when running at higher speed the increased pressure will cause the hose to "pinch" closed thus restricting gas flow.
You might want to check your fuel line filter, vacuum hoses, EGR valve, ECT, air filter, spark plugs and wires. These are the most common parts that would cause that type of problem. If the "check engine" light is on, take to the nearest autozone and they can they tell you exactly what is wrong..
Trash or debris has plugged the fuel filter. Replace the fuel filter, use starter fluid to get it started.
It could be either a bad fuel filter or fuel pump or it could be bad spark plugs or cap and rotor or spark plug wires
