1850's
First, it should be noted that women throughout history (and even in Bible times) worked as midwives, delivering babies. But the first woman to receive a medical degree in the USA was Elizabeth Blackwell, a British-born woman who came to the US and pursued a medical education. In 1849, she received her degree (after enduring much opposition from men, who believed women lacked the strength and intelligence to be doctors). Blackwell proved them wrong, but it was not easy. Many people still distrusted a woman as a physician, and it was not until 1852-1853 that she was able to start a successful practice in New York City.
Not directly, but she did hire people to commit acts of piracy.
